Questionnaire

Hi, thank you for being willing to participate in this questionnaire.  This questionnaire is not for pay.

I am a local Christian author doing research on attitudes of Christians about the body and sexuality in preparation for a book I’m writing.  If, after reading through this questionnaire, you are still interested in participating, please answer these questions to whatever degree you feel comfortable doing so, and then please email your response to timothydeanmills@yahoo.com

The subjects and questions in the questionnaire are very explicit.  If you are easily offended please do not even bother reading them.  But if you are fine to read and answer them, the more specific your answers can be, the more helpful they will be to me.

Your answers will be used anonymously, even if I have your name.  No one’s name or identifying details will be used in the book in any way.  This questionnaire is about getting a sense of what people are thinking about these topics, and to make sure I’m covering all the important issues people would want addressed in a book like this.  The more people who can help me brainstorm this, the better.
